I've often (quietly) cursed this particular item simply because the belt invariably comes out of the contraption and spends more time out of it and only goes back when I put it back. It's on the 4.2 3-dr and I can't remember if the 5-drs had it.
But my underlying question is, "why did Toyota fit it when it patently doesn't do what it's suposed to?"

I guess it’s supposed to assist the driver reaching the seatbelt as it’s so far back on a 3 door ..don’t recall any problems when i had my two 3 doors 🤔🤔 Is it hard to move backwards and forwards so it stays forward almost handing you the belt as you get in ?.....maybe it’s gone limp 😳😳

What, in your view, is it supposed to do? I can't see from the picture quite how the belt can slip out but perhaps there's a slot at the back that I can't see. On a 5 door RAV (e.g. mine) there are clips for the rear seat belts to hold them out of the way when using the car in "freight mode" with the seats down. Maybe, on a 3 door, these are provided so that you can clip the belt out of the way when someone is climbing into the back?

I assumed that it was supposed to keep the belt straight and tidy but it comes out more often than not and the belt can sag and bunch-up. It does have an opening at the back on the side nearest the pillar to allow thw belt to go in. You could be right about the rear passenger entry though because when it works, the belt stays tight to the pillar. But it comes out more often than not.
Following Bob's logic, perhaps I need to tighten up the mounting bolt a bit?
