A mini how to (taking no responsibility if you decide to do any of this yourself!);
On ebay i ordered this stalk (thanks to Anchorman and Bob for suggestions), from the US it took around 2 weeks to arrive. I was pleasantly surprised when I opened it to find official Toyota and not a pattern;

First of all disconnect the battery. Having done the fitting now i am not sure this is totally necessary however it feels safer as you are moving around the airbag. Once disconnected leave for a minute or so to be sure!
Now locate the two circular holes in the sides of the steering wheel - roughly in the 4 and 8 o clock positions. Using a hexagonal screwdriver head loosen both bolts off fully. they wont come out as they are retained in the wheels skin. They may be quite stiff initially but will loosen off.
Then grab the bottom of the airbag (where the horn logo is) and pull towards you. This should free the airbag unit from the wheel completely. Without disconnecting the cables from the airbag unit place it on top of the wheel. There is enough slack to do so. You should end up like this;
In the centre top of the wheel you can see the plug - the empty one on the right is where the cc stalk will plug into;
Now to make your cutout. Having looked at what others have done i decided to use a circular cutting drill piece as it is only soft plastic. A 25mm bit is enough for the stalk to fit through;
Working out roughly where the centre needed to be from the other pics i slowly went about making the hole;
It is worth noting that at that point i was using a 22mm piece which i realised wasn't going to be quite big enough, so the 25mm made the circle slightly larger.
You can tidy up the edges of the hole as you see fit. Now thread the stalk behind the metal frame and through the newly made hole. It will be tight through the hole due to the angle but it will go through nicely. The hole looks quite big in pics but really isn't when you see it on the wheel. Now plug the CC stalk plug into the socket and push the cable against the wheel skeleton. In some wheels there is a retaining piece of plastic you can use but i didn't have that;
At this point it may be worth briefly reconnecting the battery, switch the ignition on one click and press the button on the end of the new stalk. If connected properly the "Cruise" light will appear next to the 4 on the rev counter. If it works disconnect the battery again and secure the stalk with two M5x10mm screws, ideally something with a low profile head.
Place the airbag back by lining it up then click it into position by gently pushing in the area of the horn logo. It should click clearly then it will be held in place. Now screw the two retaining hex head screws back tightly to secure the airbag.
Reconnect the battery and go for a drive! Having tested it i realised the hole i made is the perfect size top to bottom for the amount of travel needed to use the switch.
Took me around 30 mins from start to finish, and that was taking it very slowly and carefully and taking pics. If your wheel has the hole already or is the later design with a removable panel Mr T will fit the stalk for £54 inc VAT.
